For the Head of the Civil Registry Office to register a foreign civil status record, the applicant must provide the following documents:
✓ the original birth certificate, marriage certificate, or death certificate;
✓ a certified translation of the birth certificate, marriage certificate, or death certificate (translations are not required for documents issued on multilingual forms from EU countries);
✓ a document confirming the applicant’s identity and citizenship.
The person to whom the foreign civil status record pertains may request its entry into the Polish civil registry, including:
✓ a birth certificate;
✓ a marriage certificate;
✓ a death certificate.
